Treatment Visit Rate: $65–$95 per visit, depending on licensure, credentials, and whether the provider is an SLP or SLPA
Position Overview
Matrix Home Care is seeking a compassionate, dependable, and qualified Speech-Language Pathologist or Speech-Language Pathology Assistant to provide per diem home health services in the Miami, FL 33054 area.
The clinician will provide speech therapy services to adult and pediatric patients in their homes. The anticipated schedule is approximately three visits per week, with the opportunity for additional visits as patient needs increase.
The ideal candidate will demonstrate strong clinical judgment, clear communication, timely documentation, and a commitment to providing individualized, patient-centered care.
English fluency is required. Bilingual Spanish skills are preferred but not required.
Primary Responsibilities
Complete speech and language evaluations, as permitted by licensure and scope of practice
Develop and implement individualized plans of care
Provide speech, language, cognitive, communication, voice, and swallowing interventions as ordered
Provide treatment services to adult and pediatric home health patients
Monitor patient progress and modify treatment plans as clinically appropriate
Educate patients, family members, and caregivers regarding treatment strategies and home exercise programs
Coordinate care with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary team
Complete accurate and timely clinical documentation
Report changes in the patient’s condition and communicate concerns to the appropriate clinical team member
Follow all agency policies, professional standards, physician orders, and applicable state and federal regulations
Minimum Qualifications
Active Florida Speech-Language Pathologist license or applicable Speech-Language Pathology Assistant credential
Current CPR/BLS certification
Valid driver’s license
Reliable transportation
Current automobile insurance
Excellent clinical, communication, organizational, and documentation skills
Ability to work independently in patients’ homes
English fluency is required
Bilingual Spanish skills are preferred
Home health experience is preferred; qualified new graduates are welcome to apply
Speech-Language Pathology Assistants must work within their authorized scope of practice and under the required supervision of a licensed Speech-Language Pathologist.
AHCA and Compliance Requirements
Applicants must meet all applicable Florida home health, agency, and AHCA employment requirements, including:
Successful Level 2 background screening
Current professional liability insurance
Current CPR/BLS certification
Current medical physical
Current Florida professional license or credential in good standing
Completion of required Alzheimer’s Disease and Related Dementia training, as applicable
Compliance with employee health screening, immunization, competency, and documentation requirements
Completion of all required onboarding and credentialing documentation
Under Florida law, this role is subject to background screening requirements under Chapter 435, Florida Statutes, including fingerprinting and verification of employment eligibility before assignment.
