LSF is looking for a talented SNAP® Case Manager who wants to make an impact in the lives of others.
Purpose and Impact:
The SNAP® Case Manager is responsible for the day-to-day case management and system navigation of SNAP®.
The SNAP® Case Manager postions serve an under 12 population, a youth justice population or a dually served poppulation. Service population will be assigned based on background and skill set.
Dually served Case Manager will be working directly with youth ages 12-17 who are dually involved or at risk of being dually involved in the DCF and DJJ Systems. The Case Manager will run groups for youth ages 12-17. Youth Justice Case Manager works with youth already invovled with DJJ.

Education:
A Bachelor’s degree in Social Work or a related field.
Experience:
1-3 years’ experience with structured groups and parent education groups is required; community/agency liaison experience is desired.
