As Care Coordinator you will provide personalized care and support to adults with intellectual and developmental disabilities in our Day Program. Care Coordinator will play a vital role in helping individuals live fulfilling lives by encouraging independence, promoting engagement in community activities, and delivering high-quality, person-centered support. As a Care Coordinator you will be both an advocate and a companion, helping individuals achieve their goals in a supportive and respectful environment.
Key Responsibilities:
Ensures all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible
Ensure all documentation is accurate and complete according to regulatory requirements
Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed
Provides support, assistance and supervision to meet the needs and goals of the individuals according to their plans
Provide companionship and engage clients in social activities to enhance their quality of life
Submit daily notes of client interactions and other updates
Monitor clients' health by observing vital signs and reporting any changes or concerns to healthcare professionals
Collaborate effectively with families, healthcare providers, and social workers to ensure comprehensive care
Requirements:
Must be at least 18 years old and legally authorized to work in the U.S
Valid Driver's License
Minimum High School Diploma/GED or Degree
Must pass a background check
Previous experience working with individuals with disabilities is a plus but not required
Must be Bi-lingual (Spanish, Creole and English)
