Licensed Practical Nurse (LPN) – Home Health Agency
Location: Miami-Dade County, FL
Legacy Health Care LLC is seeking a compassionate, dependable, and experienced Licensed Practical Nurse (LPN) to join our growing Home Health team.
Responsibilities:
* Provide skilled nursing care in patients’ homes.
* Perform patient assessments and document findings accurately.
* Administer medications and treatments as ordered by the physician.
* Monitor patient condition and report changes to the RN Case Manager and physician.
* Educate patients and caregivers on disease management and medication compliance.
* Maintain timely and accurate clinical documentation.
* Collaborate with physicians, therapists, HHAs, and office staff to ensure quality patient care.
Requirements:
✔ Active Florida LPN License
✔ Minimum 1 year of nursing experience (Home Health experience preferred)
✔ Current CPR Certification
✔ Reliable transportation and valid driver’s license
✔ Professional appearance and excellent communication skills
✔ Ability to complete electronic documentation accurately and on time
✔ Knowledge of Medicare Home Health regulations is a plus
